• Photocopy of Birth Certificate (NSO Authenticated)
• Four (4) pieces 1x1 picture with blue background (Photography Service is also available inside the campus)
  
• Color Coded Envelop (Available at the Commissary Office)
• Stamped Envelop (Available at the Commissary Office)
  
• Entrance Examination Results (Entrance exam fee is Php 100.00 to be paid before taking the entrance exam)
